Senior Advocate the Graceful Aging Program services are included in ElderCare's hourly fee at no additional charge.

ElderCare's Graceful Aging Program is another way we go beyond ordinary to extraordinary.

 

Our goal is consistency and continuity of care.

 

After the Senior Advocate's initial assessment, each client and/or their family is offered a detailed plan of care, including recommendations and referrals.

 

This includes all or any part of the following:

  • Determining safety concerns and falls risks in the home and then finding the best solutions (including working with contractors and overseeing the work)
  • Assessing any medical equipment needs and working with the physician’s office and medical equipment company or contractor
  • Researching community resources for specific needs

The Senior Advocate will will continue to manage an individual’s care — called a Graceful Aging Program (GAP). This includes meeting regularly with you to monitor needs and care. Your Senior Advocate will also monitor your caregiver and the care provided in your home on a routine basis. These visits make it possible for us to review in-home documentation, reassess your care and condition, update plans of care as well as communicate to any approved family members or other contacts.

  • Paperwork organization
  • Financial/Bill paying
  • Community resources
  • Social life

At ElderCare 4 Families, a Senior Advocate meets with each and every client and/or family prior to service for a complimentary assessment. This first step in the Graceful Aging Program ensures we understand your needs.

 

ElderCare's Senior Advocates are health or human services professionals that help families care for their loved ones. This help can be through coordinating care, identifying problems, recommending solutions, advocacy and much more. Their goal is to encourage as much independence as possible while meeting the client's needs.

Senior Advocates are professionals. They are members of the National Association of Professional Geriatric Care Managers. They are experts when it comes to the aging process. Their compassion and knowledge will help you manage the growing and complex demands and responsibilities of caring for yourself or your loved one. With personalized short-term solutions and long-range plans, we can ensure safety, independence, and a high quality of life.

 

The Senior Advocate will learn about your likes, dislikes, health history, family and everyday routines. The information helps us select your caregiver by skill set, availability and personality.

 

Our goal in matching you and the caregiver is consistency and continuity of care. If for any reason, you and the caregiver are not a good match, we will immediately replace the caregiver with a new one. We welcome all feedback — positive, neutral and negative. We strive to continuously improve our services and the outstanding care we provide.
