Independence is what we strive to provide for our clients. When given the choice, most of us enjoy the comforts of our home and would like to remain at home even if it means getting a little extra help. Practical Care & Elder Care Solutions works with clients to offer a helping hand when needed in familiar surroundings. We provide the care our clients need to maintain their independence for as long as possible. Practical Care & Elder Care Solutions is committed to ensuring that our clients have the opportunity to age with dignity, have choices in managing their own lives, and remain active and productive members of their families and their communities.
Geriatric Care Management
We have Geriatric Care Managers (GCM) who meet each prospective client free of charge to determine how we can best help them lead happier, healthier lives. Our GCM will routinely monitor each caregiver and client to ensure we continue to provide premium care. Also, they can assist families by monitoring home safety, obtaining medical equipment, providing family feedback of on going changes and assisting in searching for community resources.

Also, they are closely monitored by our telephony system that verifies arrival and departure times and ensures accurate billing for you, our customer.
